Game Incident Report
At every level of play, when a player or coach is disqualified for any reason, one of the game referees must send an official Incident Report to the Director of Discipline and Mediation immediately following the game (and within 24 hours at the latest).
A report is then sent to the league for appropriate disciplinary action, if any.
It is important that all disciplinary issues be dealt with in a timely and consistent manner.
Grievances -- If you would like to file a grievance with the EBOA, please complete the form above (please specify "Grievance"). The issue will be dealt with in a timely and professional manner.
Complaints -- Like most people, referees are not perfect. A referees might act in a manner that is inconsistent with the professional standards expected of them. Rather than using abusive language or shouting at a referee - which is a breach of every league's Code of Conduct for coaches, players and spectators - please take a moment to complete the form above (please specify "Complaint"). We value your opinion.
Compliments -- Like most people, referees are human and appreciate a nice compliment from time to time. If you would like to congratulate a referee for a job well done, please take a moment to complete the form above (please specify "Compliment"). We strive for excellence, and your compliments help the cause.

Evaluators use this form when evaluating the performance of EBOA referees
